Dependent Passport Program
Dependent can take advantage of the Coventry Health Care of Florida Dependent Passport Program. The Passport Program provides access to the Coventry National Network for dependents outside of the Coventry Health Care of Florida HMO service area*.

*Behavioral Health Services are not provided through the Coventry National Network, therefore members utilizing covered Mental Health and Substance Abuse Services may be required to return to the service area.
Transition of Care
Coventry takes pride in ensuring the delivery of excellent service to our members. To assist you and provide a smooth transition to our plan, our Medical Case Management department needs information regarding your medical needs.

As we work through your transition needs, your physician may need permission from you to release your medical records to our Medical Case Management Staff. For your convenience, we have provided a Medical Records Release Form you may submit to your physician.

WellBeing Discount Fitness Program
Most of us know that regular physical activity can improve your health and well-being. This program is designed to empower our members to take a more active role in improving their personal health. Coventry WellBeing Online
Nothing is more important to Coventry than our members' health and wellness. Members have access to Coventry's WellBeing online health and wellness portal, including a Health Risk Assessment questionnaire, online coaching programs such as tobacco cessation, blood pressure management, cholesterol management and others. To help members with their program and track progress, we offer additional tools such as a step tracker, a restaurant guide, and BMI calculator.

WellBeing CAM Program
With the Complementary Alternative Medicine (CAM) Program, members have access to discounts of up to 30% for various alternative therapies including, Acupuncture, Guided Imagery, Massage, Nutrition, Pilates, Tai Chi, Vitamins, Yoga and more.
Tobacco Cessation
Online tobacco cessation support is provided to our members. The program offers members the tools to help them throughout the quitting process, including forums and chats, national directory of cessation programs to find offline local support, tailored quitting guide for each stage of change, games to distract from cravings.
Weight Loss Discounts
Members have access to discounts at Jenny Craig and Weight Watchers for weight loss programs.
Weight Watchers offers our members exciting weight management options at a discount. Available in South Florida only. The program features a $15 registration fee and a $10 weekly Membership Fee.
